Transition and Discharge

Many children’s services end their support when a child is aged between 16 and 18. In contrast, Pepper nurses offer to continue to care for a young person up to 19 years of age, supporting them through the transition into adult services.

The nurses will work with a young person and their family, and all relevant professionals, to prepare them for entering into this next phase of their lives.  This may include helping them to find other services and support they may need.
On completion of successful treatment, or if a family no longer requires the input of the Pepper children’s  nursing  team, they can be discharged.  A re-referral is an option should the needs of the child / family change in the future.
